To allow a more accurate assessment of the OG junction’s sphincter mechanism, Hill et al proposed a classification in 1996 based on its appearance when the scope is undergoing a J-manoeuvre.
Hill I – Tight closure of the gastro-oesophageal valve around the scope
Hill II – GOJ valve less tight, with respiration-dependent incomplete closure of the cardia around the scope
Hill III – GOJ valve hardly present and no closure around the scope
Hill IV – GOJ valve no longer present with permanent opening of the junction.
